Learning from Incidents (LFIs)

Learning from Incidents (LFIs) are short, shareable summaries of incidents and near misses that capture what happened, why it happened, and the actions that prevent it from happening again. Sharing these lessons across everyone who works at Timewise sites is one of the most effective ways to stop the same incident from recurring somewhere else.

Why LFIs matter

An incident at one site is a warning for every other site facing similar work. When the lessons are shared openly and acted on, a single event becomes an opportunity for the whole contractor community to get safer — rather than a lesson that has to be learned the hard way, more than once.

What an LFI covers

• What happened — a clear, factual account of the event.
• Why it happened — the underlying causes, not just the immediate trigger.
• What we learned — the specific actions and controls that prevent a recurrence.
• Who needs to know — the tasks, trades, and sites where the same hazard could exist.

Your part

• Report early. Report incidents and near misses promptly, including the ones that caused no harm — near misses are the warnings that let us act before someone is hurt.
• Share the lesson. Review relevant LFIs with your crew during toolbox talks and before similar work.
• Act on it. Apply the controls an LFI identifies to your own jobs.
